Order 66 is carried out by the Clone Army and Offee is gunned down by Clonetroopers on Felucia. Her lightsaber was left with her body never to be seen again.

Bastila Shan , Jedi Knight turned to Dark Jedi
Bastila, Carth Onasi and the reformed Revan were captured by Darth Malak and held prisoner aboard his flagship, the Leviathan . Malak tortured Bastila, corrupting her will and turning her to the dark side of the Force. She still keeps her double bladed lightsaber with her.

Boba Fett, Bounty Hunter (no Force capabilities)
Even the most feared bounty hunter in the galaxy with no force capabilities,
did manage to outwit a Jedi somehow. Fett took the lightsaber from his unknown
prey and used it as a weapon and tool for himself. Feeling cocky, Fett saw
fit to tangle with Darth Vader while on a bounty to capture Han Solo. For
obvious reasons, he lost the duel. Vader, feeling Fett was not worthy to hold
such an esteemed item, took it from him and placed it in his stronghold on
Vjun at the Bast Castle. The lightsaber was recovered by Tahiri along with
Anakin Solo finding Obi Wan Kenobi's lightsaber.

The Junior Jedi Knight series (EU - Novel)
Boba Fett's Lightsaber Collection
During the Clone Wars, Fett collected a few souveniers
of his own while helping the Empire to exterminate the Jedi. The fallen Jedi
these came from will remain a mystery burried in the wars past.

One of Jeric's cronies, an annoying Twelik but a Dark Force user nonetheless.
He combats using twin lightsabers either fastened together, parallel in the
same direction or separately making him very dangerous. However, he was struck
down by the newly realized Jedi, Kyle Kataran in combat. His lightsabers was
presumed destroyed.
Special Features of this Lightsaber: Forward tandem design. Can be used together or separated.

Brakiss, Dark Jedi
The young Jedi Knights Jacen and Jaina Solo tangled with the Shadow Academy on numerous occasions. The Shadow Academy was ultimately defeated, and Brakiss died in the conflict.

Swan was killed as Order 66 was carried out at the end of the Clone Wars. She and her lightsaber were lost.
